Youth transitioning out of supported social services settings when they turn 18 are at high risk of exploitation and being unhoused. They are also at high risk of increased alcohol and illicit substance misuse. Our KEY program seeks to proactively identify suitable accommodation for youths turning to adulthood and making sure that they do not only get housing support but are also connected to other resources to facilitate the transition at the crucial time of their development.
We proactively work with organizations that support youths at point of transitioning to adulthood. Our proactive approach allows us to start working with youths before they leave supported living arrangements mainly under social services. This allows proper needs assessment and time to develop rapport with the youths in transition. We would then co-create a care plan and groups.
We support our clients with in-home nursing needs, allowing them to receive the care they need
within the comfort and familiarity of their own home. Recovering or receiving care in the
comfort of your home has tremendous advantages, including using your own bed and chosen
furniture, enhanced privacy, and the freedom to maintain personal routines and
dietary preferences – all while receiving personalized care tailored to your individual needs.
We are also approved for direct billing with major insurance providers and manage all billing
and service delivery logistics to ensure a seamless experience for our clients and their families.
In addition to nursing care, we also offer a homemaker program designed to provide extra day
to-day support, including cleaning homes, meal preparations, grocery shopping, medication pick
ups and even accompaniment on trips and holidays, so that there is always support whenever you
need it.
Choosing to go to detox takes courage, commitment, and determination, and it is a significant
step in the healing journey. Unfortunately, long wait times for residential rehabilitation treatment
after completing detox can present a challenge that significantly increases risk of rapid relapse.
Our post detox/pre residential treatment is designed to reduce the risk by ensuring that
participants preserve their sobriety within a safe and supportive environment, as they await the
residential rehabilitation bed.
